Primary Healthcare Associates (PHA), a premier multi-specialty medical group of physicians in the Chicago south suburbs seeks a physician leader to be the Medical Director of Hospice/Palliative Care. Ideal physician will have 3-5 years of leadership experience with experience managing a Hospice and Palliative Care Program. Physician must be fellowship trained and must be BC in Hospice/Palliative Care, and must hold active licensure in Illinois.
The physician leader will serve as the Medical Director of Hospice and Palliative Care Services, assuming responsibility for the clinical/medical coordination, development, evaluation and supervision of both programs
Physician will help nurture and significantly grow an already well-established hospice program and will assist in the lead in the organizations’ effort to build Palliative Care as a program
The Hospice and Palliative Care Medical Director will collaborate/partner with a multidisciplinary team of highly skilled, compassionate professionals committed to providing individualized care to their patients and will participate in the establishment and implementation of the plan of care
The Medical Director will assume primary responsibility for monitoring and compliance of the hospice and palliative care programs with all applicable standards, regulations and requirements as well as providing in-service training for staff. Peer-to-peer consultation, face-to-face assessments, and communication with referral sources when applicable
Approximately 50% of their time will be spent on hospice direction, 30% of their time on the development of the Palliative Care Program, and 20% will be spent on OP consults
