Serve as a member of the leadership team for this Agency.
Assume overall responsibility for oversight and management of the clinical services provided by the interdisciplinary team (IDT) within the homecare and hospice house setting.
Assists with patient management by communicating information to patients, families, and hospice team.
Coordinate and delegate all aspects of the patient referral and admission process.
Assist in the development of agency goals. Develops, recommends, and administers Agency policies and procedures.
Supervise all clinical staff including RN, Hospice Aides, Social Worker, Spiritual Counselor, Bereavement Coordinator Volunteer Coordinator and Clinical Nurse Manager.
Provide information for inclusion in written performance evaluations for all direct report staff and submit written performance evaluations in accordance with policy requirements.
Oversee maintenance of nursing staff including recruiting, hiring, orienting, and training nurses and aides.
Maintain quality of care by coaching, counseling, educating and disciplining employees as well as monitoring, and appraising clinical staff performance.
Promote patient’s independence by collaborating with the patient and family surrounding their goals.
Ensure HIPAA compliance by monitoring confidential information processing.
Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks and participating in professional societies.
Maintain a cooperative relationship among health care teams by communicating information, responding to requests, building rapport, participating in team problem-solving methods.
Participate in clinical admin on call to provide clinical support
Serve as point person for EMR and pharmacy maintenance.
Overall oversight and responsibility for the Quality Assessment and Performance Improvement (QAPI) program.
Participate in staff selection and orientation of new personnel as appropriate.
Receive, investigate and make recommendations regarding patient complaints in accordance with Hospice of the Fisher Home policy and procedure.
Serve as a facilitator (or assign a designee) for nursing, medical and other students seeking learning opportunities at the Hospice of the Fisher Home.
Communicate all pertinent clinical and administrative information to the Executive Director and other members of the team as appropriate.
Participate in community and professional education regarding end-of-life care and hospice services.
Serve as a liaison for community health partners including assisted living facilities, long term care facilities, adult foster care homes and other living facilities in which the Hospice of the Fisher Home provides services.
Hospice of the Fisher Home is an independent non-profit hospice organization located in Amherst, MA. The Fisher Home is made up of a team of visiting nurses serving our surrounding communities, as well as Western Massachusetts' only 9-bed residential hospice facility.
