Job Summary:
The Manager of Service Coordination & Performance Improvement is responsible for managing daily operations and driving performance improvement efforts related to the Care Navigator service coordination model, utilization review, and the hospital operating system to achieve organizational throughput and efficiency goals.
Essential Functions & Responsibilities:
Responsible for the recruitment, selection, orientation and retention of delegated staff.
Responsible for optimizing organizational patient throughput and service coordination activities by fully leveraging Care Navigator software and roles.
Collaborates with leaders, physicians, nurses, allied health professionals, and support staff optimize patient throughput for the organization.
Provides leadership and strategic direction for service coordination model initiatives and outcomes throughout the healthcare system.
Ensures service coordination activities comply with established policies and procedures, standards of care, regulatory requirements, budgetary parameters, and strategic objectives.
Collaborates with facility and department leaders to sustain the integrity of the Care Coordination Model--roles, standard operating procedures (SOPs), Care Logistics operating system software, SNAP, POP and Escalation Huddles, and Operational Rounding.
Plans, organizes, and tracks service coordination, utilization review, and Care Navigator performance improvement efforts to ensure alignment with organizational initiatives.
Monitors and reports performance and outcome data to administrative leadership, medical staff, and department leaders.
Partners with internal and external organizations to plan, develop, implement, and evaluate UR processes and outcomes within the Care Coordination department.
Provides utilization review (UR) quality assurance and Milliman Care Guidelines (MCG) support and/or training for Care Coordination team members as needed.
Represents the Care Coordination team in meetings and committees as required.
Performs other duties as assigned.
Qualifications:
Valid RN License from Virginia or reciprocal compact state required.
Bachelor's degree in Nursing (BSN) from an accredited nursing school required.
Minimum of 5 years acute care clinical experience required.
Strong organizational and quality management skills with ability to handle multiple, competing tasks and priorities required.
Experience with computer technology, specifically experience with Windows based programs, e-mail, and Microsoft Word required.
Experience with performance improvement methodology and tools preferred.
Experience with utilization management preferred.
Milliman Care Guidelines Certification preferred.
