The hours of operation for our practices are Monday-Friday, 8am-5pm. Your primary schedule will be during those hours, with periodic need to work outside your schedule to serve our patients. What does a Manager of Population Health Clinical Services do? Under the direction of leadership, the Manager of Population Health Clinical Services will oversee tracking and monitoring of population health clinical services, work with and across a variety of disciplines to implement and support population health clinical services initiatives, and function as a liaison across the system. What does a typical day on the job look like? Oversees execution, development, and Key Performance Indicators (KPI) for all clinical services within population health including care management, remote patient monitoring, ER utilization reduction, and readmission reduction. Oversees rollout of technology and people, process, and technology functions to implement clinical services. Works with and across a variety of disciplines to implement clinical services to improve patient care and meet the organization's strategic plan for Population Health. Responsible for talent life cycle management, including recruiting, hiring, onboarding, orientation, mentoring, development, engagement, retention, performance management and time keeping. Provides implementation support for population health initiatives, inclusive of committee development that supports clinical services across the system, cross-functional. Supports other Population Health teams, including quality, risk coding, and other organizational groups (e.g., IT, Performance Improvement). What are the requirements for the job? Bachelor's degree in nursing; Master's degree in nursing preferred Current Missouri Registered Nurse license Minimum of 3 years management and population health experience required; 5 years' experience in both preferred. Displays a high level of flexibility, adaptability, and organizational skills. Excellent oral and written communication skills with a patient centered mindset.
